As the property owner or assigned representative, I hereby acknowledge and understand that, despite every effort made during service work provided by Carr's Plumbing, Heating & Air, and Maintenance located at 3815 W. Douglas, Wichita, KS 67213, there are circumstances beyond their control that may result in damage to my property.

I am fully aware that several factors are involved with the requested service, which may increase the likelihood of property damage. With this understanding, I willingly assume all associated risks.

I hereby release Carr's Plumbing, Heating & Air, and Maintenance from any and all reasonable liability and legal obligation to repair or correct any property damage that may occur during the course of my requested service.
